Perthes - written communication

hi evryone.
im after some help!im a physiotherapist student 3rd yr and iv been askd to write a letter to the GP who referred a 6 yr old boy with perthes diesease regarding his abilities an difficulties.Also to teacher regarding his abilities, difficultioes, advice on activities for school bearing in mind the pressure on the school day and home life.

im unsure of the layout and style of writing and also what is eminent to include.The pt has occassional hip pain, walks with a limp and has reduced ROM.

any ideas MUCH appreciated.
