Angina Pectoris Questionnaires

Hi all

i'm looking for questionnaire for angina pectoris
I know of two but can't put my hand on them....
1. Seattle Angina Questionnaire (SAQ)
2. WHO Rose Angina

and to add to that -

Is there any scale that assess the fear of the patient
in angina pectoris condition?
