Yippeee! My 1st knee posterolateral corner injury patient!

Interesting case.

Got a female patient refferred for 'right knee athralgia'. This was 3 months after a fall on a bent knee onto a concrete surface. She had mild swelling then and reports getting a knee support, analgesic/antiinflammatories back then but the pain has been getting steadily worse. Problems also aggravated at work where they sit on high stools at the service counters. Pain on supporting heels on the foot rung.

She had mild pain on flexion end range, full extension. Excrutiating pain lateral knee on crossing her right knee over the left (fig 4 position). Got a ssharp pain on the lateral knee when she touched or pulled in heel just behind lateral malleolus (sural?), Pain on varus stress to the knee. Otherwise no pins & needles or numbness.

Very bad myofascial pain peroneals, gastrocnmeus(lateral head), soleus. Also plantar fascitis.

(so after reading it I thought yippee! my first plc injury patient! I hope I hadnt just missed any over the years though!)

but was wondering if it was common for the myofascial pain/trigger points and plantar fascitis to occur with this injury. Patient didnt have these problems before.

I did ultrasound and strapping for the lateral knee then prescribed a brace with hinged support. Trigger point massage and stretches for the calf muscles. Same for the fascitis which have become better. No change for the lateral knee pain however as patient awaits ortho appointment.

Anthing else I could do?. Physio management-wise or general info

I assume that her lateral meniscus has been cleared? PLC injury appears to be high energy injury or that involving hyperextension whereas your patient fell on bent knee during which compression/shearing type injury is more likely.

Sorry for checking thread late. I assumed mensicus injury cause despite not feeling the click on testing patient complains of pain. Waiting to hear specialist view from patient cause she sees ortho this week.
