initial asessment form/template
Hi
I am trying to modify/design a new initial assessment form as the one at the clinic I will be working at is pretty outdated. I was wondering if anyone would happen to have an electronic version that they wouldn't mind sharing. I am looking for ideas to better our existing one to make it more clear & efficient, yet not leave out any important aspects. Any help would be greatly appreciated!

Take a look at the preTherapy physio assessment from from Performance Physio - Clinical Assessment Collection (CD Version) You can gauge what's on there from the screen shots.

wow, replying to threads is tricky! Take 3... I've also been asking around for ideas on physio initial assessment form design too. I work in Ethiopia, where physiotherapy is still very new. Haven't had much luck getting ideas to date - the CD physiobob linked to is definitely beyond Ethiopian price ranges! Anything would be helpful, whether general or more specific to ortho, neuro, cardio, spinal, paeds, etc. Best help I've received was an APA form scanned and sent by a friend home in Australia. Can't figure out how to add attachment here, so will try pasting it into the email...

Maybe if you send a request via The Physio Shop they might send you a link to download a free .pdf as an example to get you going :)

Aha! That's what I expected - but add in Ethiopia's power cuts, lost Internet connection - I had logged in but then somehow I wasn't (so what I wrote disappeared) - tried attaching a file but couldn't (what I wrote disappeared again) - I finally succeeded to post after 3rd rewrite...
Inserting image into the post didn't work. I've found an 'attach' button, under 'advanced', but file size is limited - 3.7MB is too large.
Thanks for your suggestions. Appreciate the articles you've posted on other threads, too Physiobob. Especially the American Spinal Injury Association's assessment form.

Thanks physiobob, looked at the preview and it looks like a good form. I'm currently working on one for myself, certainly not as colorful as that to save on printing costs haha but will happily post it on here for all to judge/use once its done if someone reminds me in a couple of weeks.
