As far as my experience reaches, Stabilisation has a very important role is recovery. Manual or not in a static way, without movement. And later on in a dynamic way in different, corrected middle-positions of the schoulder/neck and with vatiaty of bodypositions.
If surgery is not an option. I would leave it alone. When settled, stabilisation and manual traction can certainly help. What ever is chosen, it is a slow process and difficult to assure the patient that there will be progress.
According to the severity of pain and degree of the disc prolapsedwe have to plan the treatment.
Since the lesion is in the c4 c5 region, traction will be helpfull with 15 to 20 degrees of neck flexion that should be static traction only for about 30 mts with accordig to the weight ofthe patient the tension kgs shouild be adjusted. if he has radiating pain or numness over the dermatomal area, TENS willbe helpfull. During the period of treatment never advise exercise prog. till the pain subsides. patient should be in rest.
Advise to the patient during treatment.
1- avoid exercise
2- avoid two wheelers
3- never use high pillows while sleeping
4- its better to use bucket seat with neck support while driving a car
5- never sit before the computer, tv, for more than afew minutes.
After the pain subsides teach the cervical spine strengthening exercise.
Shoulder,shrugging,bracing,exercise.
Advise the correct use of neck movements.
