Bariatric, diabetic patient with large pouch, feels like lymph - any ideas??

i have been working with a patient aiming for weight loss - he is currently 190kg, down from 260kg last november.
he is an amazing chap, really willing to give anything a go, but is having ongoing problems with a lump. he seems to think it is a hernia, but it doesn't increase in size with an increase in abdominal pressure so i'm not so sure. he has been to see the doctor, but he was told to go away and lose weight. the problem is, it is a pouch about the size of a rugby ball that hangs from the right side of his groin. it interferes with his gait, and becomes sore. it also gets in the way during exercises. to me, it feels like fluid, and it changes over the course of the day dependant on his position. he feels like it gets harder if he sits for long periods.

has anyone had experience with this type of thing? possibly lymphoedema therapists? i thought about trying a garment like bike pants or something similar to give it a bit of support, but there is nothing in his size...
do you think k-taping might work? or massage?
does anyone have any guidelines RE increasing his venous return and overloading the heart?

any ideas would be greatly appreciated!
