Advice on Trans Tasman mutual recognition application please

Hello, I'm wondering if someone can give me some pointers on the Application for Trans Tasman mutual recognition form?

With regards to your proof of ID in section A if using an overseas passport (uk) do you definately need to already have an Australian visa as I was going to wait for the registration to go through before applying for a visa? Also to confirm that in section D you can use the same evidence that you have already used in section B or C?

Question 6: it asks if my residential address is the same as my principle place of practice in Australia. As I am not yet in Australia neither my residential or place of practice address is in Australia so I don't know how to answer this question and there are no guidelines on the board's website. Does anyone know how I should answer this question?

Section C Q.11: it asks for primary qualification or other method which my registration is based but is just set out to input for university degrees whereas my ability to work in new zealand has been through the overseas new zealand physio registration application/portfolio. Does anyone know how I input this in this section?
