cervical spine stretch pattern
Hi all
I am looking for some help I am studying Physio and have a project to do but cant find any info. The project is a presentation on a cervical spine c4-5 left regular stretch pattern with peripheral sensitisation and of low irritability. I would like to find out how this would present in clinic eg mechanism objective and how would you treat it. There seems to be very little info on compression and stretch patterns out there. but obviously lots on nerve root compression.
any advice will be extremely appreciated

since it is of low irritablity your project fits more into the frame work of an ANR (Adherent Nerve Root) or positive neural tension.Since you have mentioned that it is on the left side, the patient should produce pain in the end range of (R) lateral flexion, either pure or added with flexion causing peripheral sensitisation. Once the cervical spine is in neutral position the peripheral sensitisation is relieved. On examination the neural tissue provocation test should be positive for median & to the radial nerves. The treatment would include neural mobilisation. Refer to neurodyanamics by Michael Shacklock.
Single facetal dysfunction at C4-5 can also give you stretch pain. If there is extension dysfunction on the left, there will be pain on (L) lateral flexion with peripheral sensitisation. The treatment objective here would be to open the facet on the left. If there is a flexion dysfunction on the the right, the left facets will not close & the treatment here would be to close it. After the facetal mobilisation, treatment can be extended to neural mobilisation doing more of proximal sliders, as there must have been some sensitisation there.
