Proof of work experience for universities?

So, I observed the work of a physio therapist for a week, and when I asked them to write me some sort of a paper, just like a proof for universities, that I really did observe the work, they told me, that I can just write it for myself and they will put a stamp on it. I started writing it, but realized, that I don't really know what to write :D It's not a letter of recommendation, so examples on the internet do not really help. Maybe you guys have some sort of advice or even examples from your past?

maybe you could state how many days you were there and for how many hours; what kind of patients you saw and what kind of treatment techniques you observed. Reflect on timetables, time management, professional development and documentation.
Include a section on what you have learned from the observation.

There should be quite a bit o write about. :)
