Stroke Physiotherapy Paperwork

Hi,

I am a physiotherapist working on the stroke unit of a hospital in England. Myself and a colleague are currently trying to re-design the initial assessment paperwork; Streamline it and generally make it more concise, but keep it specific to stroke assessment.

Does anyone have any advice on what works well for them in their area of work? Or templates of the documentation that is used where they work?

Any help would be greatly appreciated!

In Stroke Unit we use: Trunk Control Test, Motricity Index, Barthel Index (or Modified Bartel Index), Catherine Bergego Scale, Scale for Contraversive Pushing, Rankin Scale, Functional Ambuation Categories. Good work!
