Questionnaire on Access to a NHS Physiotherapist

Hi,

I am currently a student doing an independent research project on the process on accessing physiotherapy. I was wondering if it would be possible for members of the public who have suffered or still suffering from musculoskeletal problems, therapists and other healthcare professionals to answer my questionnaire. It should take no more than 5 minutes and would be helpful on furthering my knowledge.
