Hi,
I am sure you are aware of the professional, ethical and legal requirements of doing research.
Thus - could you please state who you are, where you study, purpose of the questionnaire/ introduction of the background, supervisor/ organisation that supports your research and what is going to happen with the information you acquire.
And - a little hint - you are posting in a physiotherapists' discussion group, maybe you should move your post into the patients' group.
