Improve heart patient's performance capacity

Hi everyone,

I am a soon to be fysiotherapy student and need som help with a task about improving a heart patients performance capacity.
The task is as follows: mr. Hart comes to see you (aged 60). Three months ago he suffered a mild myocardial infarction, from which he has recovered fairly well. He has already followed a cardiac rehabilitation program, but still has pains in his chest during strenous exercise/exertion. He is on medication (p-blockers and blood thinners). He also takes Nitrobaat (nitroglycerine) for the chest pains. Mr. Hart would like to improve his performance capacity.

What needs to be considered here from a heart healthy perspective and what else is importsnt?

Would appreciate every bit of insight I could get on this topic. Thanks in advance!
