Clinic notes program? PDA, palm pilot?

Hi all,

I interested in finding a program that I can write up my patient's progress notes which is a bit more efficient than the paper and pen route.

Does anyone know of any good programs... in particular I want to know if there are any programs available out there which can be used on a PDA or a palm pilot for the benefit of portability!
