I just had a very interesting training session with a Stroke Therapy team today, and the beauty of the situation was that how on the spot certain exercises were employed specific to the patients problem.
This patient was hyper-extending at the knee, and accordingly with research guidelines indicating that stroke patients require constant repetition, appropriate exercise via facilitation was a key goal, while this very interesting stepping maneuver was created to prevent hyper-extension.
Interesting stuff, and indeed it would be useful if ideas like these when made up on the spot were logged.
I believe an online database would be most useful:D

This free database was originally set up for exercise suggestions for Spinal Cord Injury clients but through grants has expaned considerably to be more general for neurological clients. Like many of these modalities, each exercise should be considered in depth for it's own merits and application to each client. Luckily they have built in the ability to modify and personalise the attached script so you can ensure that the instructions specifically suit the client's needs and abilities.
