Physiotherapy Referral in Private practice

I just wanted to know your views and ideas regarding creating the most appropriate suggestions, ideas or an ideal format containing the main conditions, disorders, areas of medicine or physiotherapy that need a physiotherapy referral to be sent to a Physiotherapist. So what suggestions or format you will give to your referring consultant, clinician or doctor? Kindly give your suggestions with any format suggested in separate words document or some compiled format. Interested to know your views.
