I have a patient that presented with altered dermatone in the L5 distribution. Pain is from greater trochanter down to lateral knee to lateral ankle. However, there was no pain on flexion or extension. O n testing the hip pain is illicited in external rotation and abduction is weak. Fabers test produce pain in the lateral thigh. THe greatest difficulty is a catching pain coming out of flexion into extension. The pain is in lateral thigh and the lateral knee. I'm thinking ITB, however there isn't pt of tenderness over the greater trochanter, just lat tibia. Another problem the patient has is initiating gait. Once they are started its ok. SI joint, ITB problem or gluteal bursitis. Does anyone have any suggestions as to a diagnosis?
