Hello all. Just wondering how many physio's are out there worldwide working in post-acute care teams. Our team comprises of mainly RN's with also allied health staff (PT, OT, Pharmacist and Care Assistants). We target patients in hospital who can either be discharged home early with our support (eg post elective orthopaedic surgery; DVT's, IV antibiotics bidaily; exacerbation of CAL) or those that have multiple problems that can be managed safely at home (eg complex geriatric patients). Our care usually extends only to a maximum of 2 weeks. Our team has been in operation for just over 3 years - it's a wide mix of patients and different new issues face us everyday. Look forward to hearing from other physio's in similar teams and sharing experiences and ideas across the net. We're based in Sydney, Australia.

I know this is quite a late reply but I have only just got here. I am involved with acute community care for children in Scotland. It's a new project and I'm basically making it up as I go along but I would be interested to be in contact with anyone who has a similar experience!
