    Stream

    I am new to the forum so sorry if this has been asked before but I want to find the STREAM assessment form. I have searched google and theres lots of articles but no actual form. can anyone direct me. Thanks

    I couldn't find a form on the net either. We use alternatives in our part of the world so I don't have a copie myself

    However this link takes you to a formative paper with the STREAM as an appendix at the end.

    Reliability of Scores on the Stroke Rehabilitation Assessment of Movement (STREAM) Measure -- Daley et al. 79 (1): 8 -- Physical Therapy

    Unfortunately something bad has happened to the formatting of the PDF version but if you look at the HTML full text version there are images of the full score sheet.


    If no-one offers a better version try contacting Nancy Mayo and McGill university. She is one of the authors You will also find the STREAM covered in her book Physical Rehabilitation Outcome Measures but I am not sure if it has the form or not.

    Thanks for that. had found that one but as you say a bit of a muddled up form that I cant even print. I think I will try your second suggestion.
