    Question Diagnosis of Neural Tension

    I'm a patient who is suffering chronic and acute pain in upper and lower extremities with no diagnosis other than simple myalgia. This is despite having seen 20+ doctors and 4 very well regarded neurologists in a major American city, including at the regional medical research institution.

    A PT I have worked with in the past mentioned to me the possibility of neural tension causing my problems. I am very interested in finding a doctor who can determine if the cause of my problems is neural tension.

    However, I am unsure of what type of medical specialist to seek - is it a neurologist or some other type of doctor? If the former, I am suprised that none of them have thought of this possibility. Or, do I need to ask if they have training in some specific area, such as "clinical neurodynamics" (or some other term)?

    Any help would be appreciated, as I have gone from being a distance runner a few years ago to confined to w/c all day, every day and unable to practice my occupation of software engineer.

    You do not really provide enough information. What tests have been done and what has been ruled out? Did you have any fever or generalized illness that preceded the problem?
    Have you seen a rheumatologist?
    Have you had Lymes disease ruled out?
    I would also reccomend trying to find a physiotherapist who is an IFOMT (international federation of manipulative therapists) member and has sports physiotherapy experience to get a musculoskeletal assessment from a physio with advanced training. They should also be able to advise you if there are problems with neural irritablity and if they derive from a source the physiotherapist can treat.Ideally, I would try to find a physiotherapist with advanced manual therapy training and acupuncture skills.

    It is difficult to know exactly what your problem is as you haven't described it in detail, but if you are looking for someone who could help you with neural tension you would be better off with a physio, as it is generally not included in medical training as far as I know. Otherwise, ask if someone is familiar at least with the work of David Butler or Michael Shacklock, who are the experts.
    On the other hand, it sounds like you might possibly have something closer to fibromyalgia if it is general aches everywhere, and you may be better off seeing a rheumatologist. You have seen an enormous amount of doctors, though, and it seems unlikely to me that anyone will be able to give you a definitive diagnosis as I assume all important testing has been done.
    I don't mean for you to take this the wrong way, but from the brief history you have mentioned, it sounds either like something extremely rare and difficult to diagnose, or perhaps there isn't an easy diagnosis. Often these kind of pains can be impacted by outside things, like relationships, stress, anxiety, activity levels, etc, etc, etc. That doesn't make the pain any less real, it just means perhaps you need someone who can think quite holistically about your problem, and there is almost definitely not an easy solution such as a specific diagnosis that can be 'cured'.
    I would like to stress that I am not trying to offend you in any way, and I don't know all your history, but I work with patients with very chronic pain and it is a very complicated (and real) thing, and there is no easy solution. I'm sorry because that is probably not what you want to hear.
