There is one called the Neck Disability Index which is quite functional and comes up with a score that should indicate changes in condition with treatment. I found it, along with a lot of other questionnaires, at www.ohioproject.com/neck_num.asp
I'm searching for an outcome measure to use with neck pain patients. The only one I've briefly heard of is the Northwick Park neck pain questionnaire. Has anyone any experience of using this or any other that has been useful?
