1) No you don't need this, you just need to put the date you would plan to come and say getting your visa is dependent on getting your registration - you won't come until you are registered as you want to come to New Zealand to work as a physio and if you can't do this you aren't planning to come / can't financially justify coming / whatever your reason is.

3) I appendix numbered all my evidence then wrote the title in the table, then appendix number in the reference column so it was easy for them to find everything.
4) It cost me £60 a go, but I missed some the first time and ended up going twice. Also I think it has to be a notary which is more highly regarded than a solicitor (I might be wrong but this was my understanding when I did it)
5) You just phone up the HPC and say you want a letter of good standing. They will post this too you - just reference it in your contents and put it in - it's just a 1 page document saying your rego dates and that you don't have any convictions etc.
