Hcpc application

Hello,I am a physiotherapy student in Turkey.And I would like to register hcpc after I graduated.
My question is;In application form professional experience part,I have references from my supervisors.
How can I add them to form?My supervisors will write a reference with signatured paper and then I will add to form or my supervisors will write upon the form and sign to it?Please can anybody help me?
