I'm looking for good advice from preferably manip therapists or physios who specifically deal with patients who have nerve/chronic pain.
I have just assessed an older female who had a TAH (total abdo hysterectomy) for Ca about 2mths ago. Basically she was given an epidural at the last minute-(she said she wasn't told the full risks and is now taking legal action). She recalls the doctor trying 2 times b/f he got needle in successfully.
S+S are:severe shooting pain posteriolaterally down her left leg to her heel. Non resolved L thigh leg numbness since op. She has allodynia in her foot and heel+ and has no movt in her ankle (unable to move ankle on command-not sure if pain limiting?). Aggs- putting heel on ground when walking (using frame)-causes sharp shooting pain from heel up, knee flex in sitting (heel not on ground), Eases- having L leg up on stool, knee straight (no incr. in P when bends forward-slump position, able to touch floor).
All her lumbar movts are restricted by pain, rotation in sitting is limited to the left by pain-ok to right. On palp in sitting (no way she could get into lying)she was tender throughout LX, and shes got some gluts wasting on her L side. She has 4/5 knee ext/flex, 4/5 hip flex and feels numb in saddle area but states no problem with control of B&Bladder. She's getting some radio for her Ca.
Not quite sure how to proceed with this lady as there are multiple issues here. MAIN one pain.!. Any ideas from anyone whos good with nerve/pain stuff. I was thinking her SC/surrounding nerves have gone abit awall and her SC/DHorn has become sensitised (ie allodynia) after being irritated by the epidural. I also don't know if the epidural was done correctly OR NOT so am abit afraid of getting involved in the legal side of things????????????????????????????/
