Post Acute Community Care

Hello all. Just wondering how many physio's are out there worldwide working in post-acute care teams. Our team comprises of mainly RN's with also allied health staff (PT, OT, Pharmacist and Care Assistants). We target patients in hospital who can either be discharged home early with our support (eg post elective orthopaedic surgery; DVT's, IV antibiotics bidaily; exacerbation of CAL) or those that have multiple problems that can be managed safely at home (eg complex geriatric patients). Our care usually extends only to a maximum of 2 weeks. Our team has been in operation for just over 3 years - it's a wide mix of patients and different new issues face us everyday. Look forward to hearing from other physio's in similar teams and sharing experiences and ideas across the net. We're based in Sydney, Australia.

Clandy, We have recently (6 months ago) set up a similar team comprising of Bed Manager (nursing background), Physio, OT, Therapy Assistant, Social Worker, Dietician and Speech & Language Therapist (later two a few hours the rest full time). We aim to assess patients in A & E to prevent admission, pick up patients in the hospital (mainly medical) who could have relatively short admissions with supported discharge (ie patients 'off their feet' following UTI once medically fit, post ortho surgery). Physio and OT provide a 2 week outreach follow up. We also deal with more complex discharges aiming to prevent delays. We are based in Paisley, Scotland.
