It seems straightforward enough to me if amended to read "gain optimal function" rather than "return to optimal function". Obviously if people with learning disabilities have nver achieved optimal function then they will not be able to 'regain' it

This statement is very general and does not give specific reference to learning. It may indeed be appropriate for the outcome of rehab post stoke.
how about "facilitate individual potential"
