Hi all,
I am thinking a lot about how we can provide better integrated care, especially for people with chronic conditions, and wondered which other disciplines you regularly get referrals from, refer to and share patient information with? Is there a way we can be more joined up in how we share information between different therapists a patient / client might be working with, whether that's a doctor or a personal trainer or a chiropractor etc.
Would love to hear how you're working with other disciplines to see if there are any learnings there.

Hi,
I would recommend to start checking, where on the continuum of collaborative practice you would like to start. E.g. do you believe a multidisciplinary approach is sufficient or would you like to learn about interprofessional practice?
There is a lot of research out there for IPP (interprofessional practice) and IPE (interprofessional education), from case and clinic descriptions to Cochrane reviews.
You could also start by reading through content lists of the Journal of Interprofessional Care and see which specific areas interest you.
