Hi

I am trying to modify/design a new initial assessment form for my clinic and was wondering if anyone would happen to have an electronic version of one from their clinic that they wouldn't mind sharing. I am looking for ideas to better our existing one to make it more clear & efficient, yet not leave out any important aspects. Any help would be greatly appreciated!
