Lumbar radiculopathy

Hi,
I have been managing this patient for awhile, but i hve not seen ny much improvement. He suffered disc herniation at L2/3 and L4/5 and L5/S1 which resulted in paraplegia. He had surgery done but no improvement. He was later refered for physiotherapy. MRI recently taken revealed scar tissue formation at L2/3/4 but no impingement on nerrve roots while there is herniation of L5/S1 disc with impingement on the nerve roots. He often experiences pain on both feet along the course of L5 though this occurs after defeacation. Any ideas on what to do? Thanks
