7-item Berg Balance Scale

Can anyone suggest where I can find a copy of the 7-item Berg Balance Scale? I've got a copy of full 14-item scale, but can't find which of these items is in the 7-item scale.

Alternatively, has anyone used another balance scale with around 4 or 5 assessment measures?
