    I am Kiran P.S Master Physiotherapy (Cardiorespiratory ).While going through ur xray the AP view of it ...as such any boney pathology is not present and so difficult to comment on that ground...From ur question the exact tender point could make a diagnosis of some soft tissue injuries...any history of fall or else should checked in .

    thank you very much, i feel the pain specially when i try to rise my arm overhead and when i do that my upper arm pushes collar bone out and the pain is mostly in the front corner of my shoulder and when i rotate my arm it crack against shoulder blade. i injured my shoulder while i was tryin to put heavy box overhead to high shelf then i lost control and the box twisted my right upper arm forward and i felt that something teared in my shoulder and very sharp pain went to the centre of my head. i been to doctor and he gave me pain killers which helped but the pain never gone and i still feel my upper arm is not in its place and my shoulder not stable.
    The accident was about five years ago and i took the x ray last year and doctor said the same as you. but i just went through the net to know more and compared my x ray to normal ones i noticed that at the top of my upper arm bone head there is like bone fracture could it be the ac joint has been damaged? and what it could be my problem? thank you

    There are many causes of persistent anterior shoulder pain and it's not always a bony one. It is possbile, based on the description of your injury mechanism that you may have overloaded your bicep and caused injury along the bicep's attachement. More often it is possible to damage the labrum i.e. a rim of cartilage surronding your shoulder socket in which your bicep attaches to. A tear in the labrum in this regioin is sometimes referred to as SLAP lesion and sometimes it does not resolve spontaneously. It would be a good idea to see to a sports physician or physiotherapist for the diagnosis of this type of injury.
    Of course, there are other possibilities but the chronicity, location and injury mechanism you have described fit this particular injury the best (and yes it clicks and clunk too). I doubt your current problem is a bony one as a humeral head fracture rarely presents as a problem beyond it's normal healing time frame i.e. 6-8 weeks.
