    Brief Medical History Overview

    Disc Bulge L5 S1? Burning Feel & Pain Pls Help. I want it fix :(

    Physical Agents In Rehabilitation
    Im 18. Male. Very fit. Do alot of Road cycling & Racing with a L5/S1 Disc bulge


    This is from the Physio.


    lumbar spine movement flexible in extension lacteal side flexion right side rotation restricted slightly compared to left rotation flexion is full of pain free straight leg raise and movement of hips are normal supine he does have elevated left crest over active tender left psoas quadratus lumborum palpation in of lumbar spine reveals general restriction over the left fascists of lumbar spine clinically mechanical lower back pain these certainly evidence of muscle imbalances marked over activity of there psoas quadratics lumborum


    After this, moving onto late 2011 I had an MRI scan and found out that I have a mild disc bulge L5 s1?

    Do alot of cycling and I get a burning feel. After about 30 minutes I have a very aero position on the bike. making my back flexed. I do racing for 90 minutes. And long intense rides 2-6 hours.

    Wondering If i can get any other help on here like more stretches/Exercises anything else or what to suggest?

    Ive had physio Thepray
    Sports Massges

    Used a foam roller does help a small bit. But again.. not alot.

    I do P90X Ab ripper work out.

    Had manipulation to the back I had no pain for about 6 weeks until it came back..

    The sport massages don't really work for my lower back, only upper back/legs etc I get a little tiny bit of relief in the lower back from it not really noticeable

    Creams: I use Movelat Gel this works a bit stop inflammation a bit. But still in alot of pain with this on still..
    Is there any other creams?

    Tablets.. Tried Painkillers.. Paracetamol, Ibuprofen, Nurofen and Naproxen 500mg. None of them have affect on me. I guess as I take them I have no pain, then the pain comes after I have had them.

    I do have epidural cortisone injection sometime in March. Hoping this reliefs pain. Yet it still doesn't fix the bulge.

    So what do I do?

    back pain difficult to diagnose when you see it never mind over internet. however, seems as though you have some mechanical back pain. where exactly is your burning feeling? groin, buttock, legs, feet etc. i would not recomend ab workouts but pilates or core work as these muscles more important for stabilising and controlling your back. you mention L5-S1, has anyone assessed you sacro-iliac joint - this can affect things. also seems you need to have myofascial release type massage, if this and anti-inflammatories are giving short term relief then stabilise with pilates. other considerations may be blood tests to rule out any inflammatory component to your symptoms, see and discuss with your GP.

    the disc bulge will not go, the cortisone may help releive the inflammation and pain but if the disc bulge does reduce it will be over years.

    also check your set-up on the bike and adjust as neccesary

    you will need a sereis of exercises to do, but this depends on your exact presentation and their effect.

    where exactly is your burning feeling? in the lower back.

    What type of Pilate exercises?

    burning in lower back without any referral into legs is good, would query significance of your disc bulge on MRI. burning into groin or saddle area, or loss of bladder/bowel function would be a worry.
    pilates, see an instructor experienced with working with physios with people with low back pain, as they can decide where and with which exercises to start you on and ensure you do them correctly

    Today i used some cream called deepfreeze it felt like it was burning, inflammation just what i experience when cycling and sitting down just more intense. Back feels like its getting worse. what to do.. what to do..

    if it is definetly the cream that made you worse, ie assuming back was impr, or no different prior to you applying it and worse straight after, then you may have some altered nerve sensation around the area - which makes them hypersensitive, it may be overstretched or irritated/trapped. again difficult over net but you may require some nerve type medication such as amytriptylline or injections aroudn the nerves - very complex issue and you need to discuss with your physio and GP

    I only put it on before i went racing last night so it felt like it was burning exactly what i feel like but without the cream aswell it feels like my back hurts more when riding

    Go to a Mckenzie certified physiotherapist (Credentialed and Diploma Qualified Therapists | McKenzie Institute UK) and get an assessment to try to find out if the disc bulge is the structure causing the pain. A typical posterior disc bulge usually responds well to Mckenzie back extension exercises. The type of ab exercises in that video of yours is not going to help your disc bulge, more likely make it worse. Try prone bridges and other non-flexion based ab exercises.

    Ok back to hopsital today. I have a picture of my MRI its an L4, not L5



    Im having Epidural Injections for it whenever I get from the NHS 6-8 months properly...

    Does any of this change the exercises or know of anything more for this disc bulge?
