    Brief Medical History Overview

    Age: 23, Male, Presenting Problem Since: 4-5 Years, Symptom Behaviour: Stays the same, Symptoms Worse (24hr Behaviour): No difference, Aggravating Factors:: Twisting, lying at a angle, carrying a heavy rucksack, sitting doing nothing for a long time, Easing Factors:: Turn my body to one side then quickly turn it the other way, lie on my back on the floor, do some sit ups, and on the way down if I make sure I lie down flat on my back on the floor, No Investigations, No Diabetes, No history of High Blood Pressure, No Medications, No Osteoporosis, No Hx of Cancer, No Unexplained Weight Loss, No Bowel/Bladder issues, Other Info: Don't think so

    Major problem / Symptomatic Areas

    Thoracic Spine

    Hello everyone,

    I know this is very difficult and maybe impossible. But can you guy's have a look at the following pictures and video, to see what the cause of my mid back pain is.

    I have had pain in the middle of my back, right in the middle, for a few years now. If I am not active and sitting about it gets quite sore, but once I'm walking about and being relatively active I forget about it. It is the most sore when turning, for example sitting in the front of the car and trying to reach round to the back. Or if I am lying on my side or twisted on a bed/couch.

    The only thing I can do to relieve the pain/tightness is to turn my body to one side then quickly turn it the other way, i feel it "pop" and then the feeling is gone for a while. Or I can lie on my back on the floor, do some sit ups, and on the way down if I make sure I lie down flat on my back on the floor you will hear an audible 1-3 "cracks" and again the pain goes away for a while and I feel normal.

    I used to spend alot of time sat at a computer, more than likely with bad posture, also I sleep with one pillow (just can't sleep with two) and also sleep on my front/side.

    You are right. I can make a lot of intelligent sounding observations but without testing you with movement & position to either abolish or reproduce your pain, the pictures didn't help me to diagnose your problem. It sounds like your back likes extension and movement an doesn't like sitting or stationary positions.
    Try to do prone back press ups ( keep hips down and extend elbows) 5-10 times nd see how you move afterwards. Try this when you hurt and not

    finchr is right. In order to make a conclusion needs to know how you feel before, during and after various movements.

    However, based on the information you provided I would probably guess you have non-specific back pain, posture related. This is because from your video you look like you tend to extend your back a bit too much. When you stand / sit and use your arms, you should always think about sitting tall as if trying to elongate your spine skyward, not sitting straight. In terms of exercise, I would probably try 1) slow repeated flexion in sitting to the end of range or mild pain; 2) slow repeated trunk rotation in sitting towards the more painful side; 3) squatting against a wall with a flat thin cushion in between your low back and wall, keep the pressure of your low back against the wall during the entire movement. Keep the pressure just hard enough to prevent cushion from dropping, and try not to fall yourself too. Slow repeated movements, 10 at a time, concentrate on technique and how your back feels rather than how much you have done; 4) Heat and massage the paraspinal area to relieve excessive tension. Maybe even some cat stretching..

    Thats just my initial thought. You really should arrange a physiotherapy session to analyze your problem before doing these exercise, if thats impossible then try it with caution, and do them slowly. Sometimes initial response is more pain, but your movement improves afterward, and become less stiff, less flare ups. However, if overall response during AND after exercise is negative, then dont do it again.

    You need to be a detective here. Try 1 series of end range movements; bending forward or back, rotate towards or away from. Do 1 or 2 sets of 10 of only the 1 direction. How do you feel and where, during and afterwards? Is there a change ? if it is better you know what you need to do. If it is worse then try the opposite. When you find what makes it better do it 6-8 times a day. Always reassess. If walking makes it better then...
    You are looking for the body posture and movement that provides a lasting relief then staying away from the opposite until your body heals enough to slowly return
