    Patient consent

    Hi

    I have just set up my own practice and I am looking for a acupuncture consent form.
    Are there anyone who can point me in the right direction please?
    Do you guys ask the patient to sign this form, or do you just register that you have obtained consent in their notes?

    I print and use this one but also ask whether they have eaten in the last 2 hours, if they are pregnant and THREADS. It is very important that they sign and read it thoroughly to ensure informed consent. If you are planning acupuncture for the next session I often give they a copy to read so they have a clear understanding of the process and possible side effects.
