    Acquiring info for new practice

    I would like to start my own practice and I am trying to acquire some information or templates regarding: informed consent forms, assessment forms, policies and procedures etc. that would be compatible with canadian standards. Any help would be very much appreciated.

    I have been putting some forms together myself for the same reasons and am also pretty new to setting up a private practice. I would happily let you see what I have done in return for some feedback on them. I am in the Uk so you wouldn't be able to use the actualy forms, but it might give you some ideas...!?

    Various professional associations may also provide a set of tempaltes that can be customised for use.
