    Dying for help- old newzealand forms

    Advertise your physiotherapy job on Physiobob's Job
    Hi,
    i would really appreciate if some one could help me with this. I need the old forms for newzealand memebership. i had already done them more than half, that my little kid split water on them. i went to website to print a newcopy and realised that they have amended the form. Please if some one could send me the attachment for the old forms as i am more than half way through with the old ones.

    Could anybody please help if you have a copy of old validation of work history forms or where i could find it?i have a copy of the old application form but cannot find the vaildation of work history form on the website- if anybody could email this onto me i would be very grateful!

    There has only ever been one form.

    Hello
    I am sorry to be asking further questions which may have been answered numerous times already but I cant seem to find on the forum. In certifying my documents is it possible if its a large document to get the solicitor to write "Certified true copy of original document sighted including pages 1-100" for example or do they have to certify each individual page? I know with the new forms this is acceptable but im wondering if this is the same with the old competancies and forms?

    Thank You for reply! That makes things alot easier!! Also i have noted it says the person certifying needs to put their contact address on each page they certify!Its alot of writing for someone that does not have an official stamp with their address on it!

    Its alot of writing for someone that does not have an official stamp with their address on it!
    Yes, exactly, but it is also a legal requirement. And that's why most people who are in the position of having permission to certify documents do own such a stamp. After all, that's a very cheap investment.
