    Re: NPTE

I got my credential evaluation done from ICD this month. Can anyone please tell me how to contact New Jersey jurisdiction and get the 'application form for licensure as a physical therapist'? Can someone send me the link if known? I want to write my NPTE atleast by this year-end so it would be helpful if someone could enlighten me asap!

    Thankyou so much guys!
