ive been given a project on lower back pain. patient has had lower back pain for years. she has had treatment and has had no pain for a couple of years now. she has had a baby and is afraid if she carries baby on the back the pain will recurr. (person lives in africa where babies are carried on the back). using biomehanics to explain do you think she should carry the baby on her back. thanks.

Think about the flexion forces placed on the back during pregnancy and carrying a baby in front rather than on her back. Loading on her back is least likely to cause pain in the spine out of the possibilities (carrying baby on hip or in front) as it is a weight carried closest to the axis of the spine.
Have a look at some back care when lifting sites, they should give you some ideas of the forces of carrying loads in different body positions. Ask her WHY she thinks carrying the baby on her back will cause pain. Education can provide a lot of reassurance.
Its not the carrying of the baby on the back, its the how she, with her structure, carries her child on her back. Have you asked her to show you what she does?
Someone once told me that women in Africa can carry children up to the age of 4 on their backs, so this child could be quite heavy, is mum stong?
It would be nice if she could learn act in accordance with the culture she inhabits.
