To develop a really good database you need to include clinimetric information (reliability, validity, responsiveness, norms etc.) as well as the utility of the measure. It is a lot of work. You shouldn't just rely on opinions of other people but look at the scientific merit of the scale.
Rather than developing your own database what about using some good ones that are already available. Here are two excellent textbooks

This database will include scales wich are Evidence Based Physiotherapy (I was not asking for personal opinions) and is not a review, my idea is to make a list of them to take a look on it. Of course if you want to know more about it need to buy a book...
