Could rule out hip dysplasia or a SUFE with an x-ray.
Otherwise could be femoral-acetabular impingement. Rule out clinically with FABIR & FABER tests
Hello,
I am currently treating a 17 year old boy complaining of an insidious onset of right hip pain after 15 minutes of walking. This has been bothering him for approx 8 months and has resulted in him withdrawing from school sports. He reports that the pain is felt over his right greater trochanter and NOT in the joint itself.
His hip AROM and power is normal. However, he did report slight pain on resisted right hip flexion and also abduction - this did not reproduce the pain felt on walking. There is no tenderness on palpation of his right greater trochanter (bilat these do seem very prominent!). When he actively circumducts his hip, a consistent audible click is heard.
He appears to have a 'sway back' posture and on Ax of his SI Joint he showed a right ilial upslip. He also had bilat tight ITB and hip flexors (R>L). I corrected his pelvic alignment and applied MET to ITB and psoas. His home exercise program consisted of ITB and hip flexor stretches aswell as some gluts strengthening.
I have been treating him for 2 months now and he is no better.
Any advice/suggestions would be greatly appreciated!
Annie

Hi
there are 2 common possibilities for audible click in hip area.
1- snapping hip syndrome, where the Taut IT Band stips and moves over the greater trochanter
2- There can be a labral tear in the hip joint.
so do specific tests and examinations to rule out these 2
