    Unhappy low back pain (i need an answer)

    hi every one ,

    i have an assignement and i am confused , and i wish some one help me and ansewrs my questions .

    a person was strengthening the abdominal muscles by making straight leg raising.he felt pain at the low back,and the leg cant be raised to 90 degree. when he tried to decrease the low back pain, he made trunk flexion from long sitting position and tried to touch his toes by his fingers, then he felt pain at the back of the thigh.

    On biomechanical basis answer the following questions:

    1. what is the cause of pain at the low back and at the thigh back ?
    2. why cant the leg be raised till 90 degree?
    3. if the person asked u about the proper way to exercise the abdominal muscled, what would u advise him to do and why?

    I had similar problems, looks like Lumbar Spine is to be blamed, get CT-scan to see if there is any disc protrusion in the L5/S1 area.

    I am pretty sure there will be something there.

    Probably lumbar disc putting pressure on the sciatic nerve. Inability to straight leg raise is usually a sign of neural involvement.

    For correct abdominal strengthening do either crunchies with bent knees or better yet, crook lying, tightening lower abdominals and controlling one knee to chest at a time. Look at Shirley Sahrmann for abdominal strengthening or Dianne Lee.

    there could be some pathology of lumber.... get the Wikipedia reference-linkMRI done..

    Hi There,

    Since this is an assignment i assume you're studying physio on some level. Therefore i'm not going to feed you the answers. try going back to the absolute basics of your assessment findings and your anatomy. work out what does what, with the abdominal muscles and what structures are likely to be stretched, stressed or compressed etc and how that would affect pain with different pathologies.

    consider your nerve pathways, disc pathologies and tests you use to exclude or include diagnoses. research and logic - if you can present a logical arguement to your case (even if it's not 100 percent "right") then that is the basis of clinical reasoning and that is what assignment questions like this are designed to help you develop. Sorry but you're not going to learn as much if someone simply tells you how they would answer the questions.
