    Brief Medical History Overview

    Age: 27, Male, Presenting Problem Since: 3months, Symptom Behaviour: getting worse, Symptoms Worse (24hr Behaviour): no significant 24hr behaviour, Aggravating Factors:: cycling: worst initially, gets better after 2hrs of cycling, sitting with feet hanging >5min, coldness, Easing Factors:: massage, heat,, Investigations: xray: NAD according to doctor, No Diabetes, No history of High Blood Pressure, Medications: fish oil, magnesium,glucosamin chondroitin,msn, No Osteoporosis, No Hx of Cancer, No Unexplained Weight Loss, No Bowel/Bladder issues, Other Info: used to cycle 150-200km/wk. wish to go back to the cycling. currently, nil or very minimal cycling ~10km/wk.

    Hi I'm an amateur cyclist.

    My knee pain started 3/12 ago post competition.
    I have upgraded to a higher level for the first time, and over- exerted.

    Pain initially started from the medial side of knee ( muscle belly of VMO). then got worse the following wk, spreading to ant knee (patellar tendon).

    (I had osgood schlatter syndrome when i was a teenager, but never had a problem previously.)

    I normally cycle 150-200km/wk, currently i cannot cycle due to I/T severe pain ( when it's at the worst: 6-8/10) I was planning to cycle 500km (100km/day) in 2weeks.

    seen physio 7times over 12wks. seems to improve with physio, been doing VMO strengthening exs, soft tissue technique.

    ** on P/E,

    obs: mm wasting over VMO
    palp: slight laxity on L patella
    am: full range flex,ext
    pm: full range knee flex,ext,
    OJ: nil hip/ankle pain
    special test:
    Thomas test: good iliopsoas lengths L=R, 80' knee flex L=R, however on passive knee flex, c/o pain over VMO on L.
    ITB length: tight L ITB compared to R

    irritability: once pain flares up, last 30min+. easy to flare up.

    Hi just a few questions

    which knee are you having difficulties with?
    How long have you been cycling for ?
    how far was the competion when you experienced this pain post comp?
    any swelling?
    sleeping patterns? how do you sleep, on your belly compressing the knee etc?
    previous fractures ?

    how old is your bike? is it set at the correct height? do you do any other sports?

    some good information already provided but a little more if possible

    about 5months, i had physios before, but it helps for couple of days, then it comes back.

    no swelling, ? maybe bit of warmth, but only slightly.

    about 1.5y/o. not sure if it's in the correct height... but will be fitted soon. no other playing sports...
