    Thigh Injury

    Physical Agents In Rehabilitation
    i need some advice please (preferably from a physio but other people's will be appreciated)

    I play 11 a side football and in my last game the opposite players knee went in to the front of my thigh (very powerfully). I felt the pain immediately but i carried on playing for the last 20 mins (i was still running around 50% of the time but with a lot of pain). I've done some research on the net and know that it could be a grade 1, 2 or 3. However i'm finding it difficult to diagnose what grade my injury is (no time to go to the GP and i'm sure they'll just say rest it).

    Other useful info is that it's my fifth day since the injury and i can walk around but i can still feel the pain (like a cut/tear in my thigh), no real bruising or swelling. However if i step on it firmly its quite painful. I have a bandage around it and this seems to help. Also painful when i bend it.

    Can anyone tell me what grade i have based on the info i have given? And the best way to recover?

    I actually wrote a reply to this earlier this morning. It took me an hour, and i'm really annoyed to see that it didn't work. The gist of my message was that there are many things that can cause your pain. The most likely based on what you'v said is a quad contusion. With the contusion there is risk of rebleed within the first 10 days and also the blood can travel down and cause patellofemoral joint irritation if it's low enough. What this means is that you should be doing any sport right away, and you should be seeing a physio for specific rehab. From what you'v said I cannot know what is wrong I can only come up with a likely diagnosis that i would confirm when assessing you.
    The 4 stages of rehab of a contusion are: control haemorrage; restore ROM pain free; functional rehabilitation; gradual return to activity.
    depending on the severity and the exact muscle involved will let us know how fast you can go through these 4 stages.

    Based on what you'v said I feel you should be seeing someone to get specific rehab. Im sorry that I'm not really able to give you specific rehab guidelines without examining you. To be fair, refrain from sport and aggrivating the pain until you can get specific exercises from a physio.

    Hi

    Thanks for your reply (i actually received both your replies so thanks for taking the time).

    I was intending to book an appointment with a physio the coming week (but this may take a while, especially if they are busy). I'm stilling hoping to do so but over the last few days the pain has mainly gone (i can still feel it if i press on it) so don't know whether to gamble and start jogging again and see how i feel then??

    Friends have told me that they have had similar injuries and some have advised to ice it, massage it, keep it compressed etc. What do you think of this advice, could it do me any damage if i massage it etc?

    Thats good to hear. It sounds like it's improving well.
    Your frinds thoughts are good. (P)RICE(D) management is first aid for any strain or sprain. If your unfamiliar: R= rest I= Ice C= comprssiong E= elevation. P means either prevent or protect depending who you talk to and D means diagnosis. Forget all that however because as I said that is first aid - remember it for the future, as you don't have swelling and your now past the inflammatory phase RICE'ing is no longer really that important for you.

    All going well - If you continue to improve as you are now, you can be relatively confident that you did a contusion, They can resolve without physio relatively quickly depending on the severity.

    I suggest as always to seek help, but given you are now only getting pain by pushing on the area then I'd look at doing some quadricep stretches if you aren't already. Massaging will be fine as your past the bleeding phase now. If you can do a quadricep stretch pain free and can do a deep squat without much trouble then your very likely ready to start light jogging and building it up as you see fit to something a bit harder.

    Let me know how you go.
    And always RICE in the future if you don't already.

    Hi

    I visited a physio on Monday and his explanation was very similar to yours, that i have a contusion. He did some massaging, which is to help the scar tissue (i think). It was quite hard massaging and even though it was painful i think it has helped. He also did some ultrasound. Im seeing him once more tomorrow and he will do the same thing but he said the massaging will be even more painful. He asked me to do some light jogging and jumping yesterday and today but unfortunately i have had a chest infection so wasn't able to do this. He thinks i should be ok to train fully by Monday.
