    Brief Medical History Overview

    Age: 31, Female, Presenting Problem Since: 4 months, Symptom Behaviour: getting worse, Symptoms Worse (24hr Behaviour): only symptoms with certain activities, Aggravating Factors:: straightening leg, twisting motion, kicking motion, lunges, cycling, Easing Factors:: massage/movement of kneecap, Investigations: x-rays - no obvious problems, No Diabetes, No history of High Blood Pressure, No Medications, No Osteoporosis, No Hx of Cancer, No Unexplained Weight Loss, No Bowel/Bladder issues

    Major problem / Symptomatic Areas

    Knee - Anterior - Left

    Is it possible to be doing physio wrong?

    Is is possible to be doing physio wrong - I've been doing physio for knee pain (pain just behind the patellar that gets worse with twisting motion, straightening my leg from a sitting position and any sort of kicking motion), which initially involved squats, lunges, single leg squats and leg raises to strengthen the vmo, and honestly, in the last 8 weeks the pain has become progressively worse and worse, such that from Week 5 it was too painful to continue with lunges and single leg squats.

    I've also attempted cycling too, but that went very, very badly, and seems to have just compounded the pain.

    My physio always checks that I'm doing the exercises with good form at the end of every session and she's never mentioned that I'm doing anything wrong, but there must be something that I'm doing wrong because I have been doing the exercises really diligently because all I want to do is get back to training, but things are getting worse (much worse in fact) rather than better, but I can't work out where I'm going wrong so any advice would be greatly appreciated.

    If it's as bad as you say I'd say the content of the prescribed Physio treatment is not apropriate! it is possible that you are carrying out these exercises "correctly" but they are not the right exercises in sort/strength or in the frequency/repetitions etc. If you already have a physio you must speak to the physio about it. Not knowing all the details I'd say reassessment is very important ASAP. Who else is involved? have you no diagnosis at all? What training do you want to get back to do? Did you self refer or have a Doctors referral? Sometimes "less" is "more" and more detailed movement analysis and correction is as important if not more than single leg squats which could irritate pain sources behind the patella

    Firstly I am not sure of your age, body weight or nature of job you are involved in. What is your actual problem ? Was it diagnosed by a medical doctor ? Once the condition is diagnosed then you will have to be clinically examined before I plan the treatment. Anyway something has gone wrong when you are doing home exercises.

    Generally, physiotherapy does not hurt although initially the stretches and exercises may cause slight discomfort; this is primarily due to the fact that the body is not used to such movement; as the body adjust and adapts it will be less painful
