Osgood like symptoms in 35 year old =S

Does anyone have any ideas about this?

I do a bit of work with a semi-pro football team and one of the coaching staff has come to see me because of an injury he picked up in 5 a side (a classic grade one mcl sprain, no worries there).

Then while i was talking him through his treatment plan he said that he'd been having other problems with his knees for quite a while (over a year, insidious onset). He has extremely tender tibial tuberosities, one side slightly worse than t'other, noticable effusion around his patellar tendons which he says have been there for a while, pain on resisted quads activity, squatting etc

This is a guy who had osgood schlatters and severs as a 13/14 year old that stopped him playing football for around a year but hes not had knee pain since then until this started. Everything else was pretty normal in terms of muscle bulk etc although i couldnt go mad with ligament tests because of the mcl sprain and swelling from that. Anyone got any ideas? can 35 year olds get osgood schlatters?

i dont know what you mean. I havent analysed whats causing the condition at all, i've just assessed and given a list of symptoms, i have no idea whats causing them, was just saying its pretty similar to osgoods and was wondering if i should treat it the same way

    Hi
Ok, i'd think We treat not only symptoms as well origin of problem, which is excesive pull on ligament caused by overuse of the muscle and muscular imbalance.
what way do You treat Osgood disfunction?

    take care

    Hi Ophqui,

I know of one lady ( in her fiftees) who had Osgood Slatters as a child and has had problems with her knee on and off since. Her tibial tuberosities is larger and it can play up.
I know that applying elastic therapeutic tape (kinesio, kinesiology, curetape, easytape etc etc) can really help relieve the pain in adolescents and she has found out that it works for her as well. My tape of choice is CureTape.

    Taping
    Hi there,
OS is a traction apophysitis on the tibial tuberositiy as a result of the bone growth in teenage years not being kept up with by muscle elongation of the quads (same deal as Sever's at the heel with the Achilles insertion). In older patients similar symptoms occur due to muscle imbalance, and especially tight quads - rec fem, hip flexors and inadequate pelvis stabilisers.

To treat (assuming this IS what it sounds like it is) stretch +++ the quads and do eccentric quads exercises to strengthen the tendon. local treatments such as topical NSAID with ultrasound, patellar tendon straps (or taping) to distribute the forces through the patellar tendon also help in the short term but generally...stretch stretch stretch. your footy player also probably has tight iliopsoas and ITBs, and weaker glute meds.

    have fun.
